We are seeking a proactive and detail-oriented Remote Patient Monitoring (RPM) & Care Coordinator to support virtual patient care and chronic care management. This role is responsible for patient onboarding, ongoing engagement, monitoring patient data, and ensuring timely coordination of care through telehealth platforms.

You will play a key role in improving patient outcomes by maintaining consistent communication, managing RPM devices, and supporting clinical workflows.

Key Responsibilities

Patient Onboarding & Setup

  • Guide patients through onboarding for Remote Patient Monitoring (RPM) programs
  • Assist with device setup and ensure proper usage of monitoring equipment
  • Educate patients on program expectations and compliance

Patient Engagement & Follow-ups

  • Conduct patient outreach and check-ins at least 3 times per month per patient
  • Send appointment reminders, device usage reminders, and follow-up communications
  • Address patient concerns and escalate clinical issues when necessary

Remote Monitoring & Care Coordination

  • Monitor patient data from RPM software (approx. 100+ patients)
  • Identify abnormal readings and flag for provider review
  • Support Chronic Care Management (CCM) workflows

Medication & Administrative Support

  • Assist with medication refill requests and coordination with pharmacies
  • Maintain accurate and up-to-date patient records
  • Ensure all activities are documented properly in the system

EHR & Systems Management

  • Work within multiple EHR systems (e.g., Athenahealth, eClinicalWorks, etc.)
  • Accurately input patient data, notes, and care activities

Reporting & Data Analysis

  • Create and maintain visual dashboards and reports for patient data and program performance
  • Analyze trends and provide insights to support care improvements
  • Communicate findings and recommend process or care adjustments

Qualifications

  • Active USRN license
  • Proven experience in Remote Patient Monitoring (RPM), Chronic Care Management (CCM), or Telehealth
  • Strong background in patient coordination or medical virtual assistance
  • Experience with EHR systems (Athenahealth, eCW, or similar)
  • Excellent communication and patient engagement skills
  • Strong attention to detail and ability to manage multiple patients simultaneously
  • Experience with data tracking, reporting, or dashboard creation is a plus
