Remote Bilingual LPN/LVN – Nevada (Spanish-Speaking)
Work Arrangement: Fully Remote (Nevada license required)
Schedule: Flexible | Part-Time or Full-Time
Licensure: Active Nevada LPN or LVN (Compact licenses not eligible)
Role Summary:
PharmD Live seeks a Spanish-speaking LPN/LVN to provide remote virtual care for patients in CCM and RPM programs. The role focuses on bilingual patient education, care coordination, and clinical support.
Key Responsibilities:
- Conduct telehealth visits in Spanish and English
- Review patient-reported data and RPM alerts
- Document care in compliance with CMS and internal standards
- Collaborate with interdisciplinary care teams
- Support post-discharge and follow-up coordination
Qualifications:
- Active Nevada LPN or LVN license
- Professional fluency in Spanish and English
- Minimum 2 years clinical or telehealth experience
- Strong documentation, clinical judgment, and communication skills
- Experience with CCM or RPM preferred
- Ability to work independently in a HIPAA-compliant environment
